The best modalities for the transition to adult care of celiac patients diagnosed during childhood/adolescence and their impact on long-term quality of life and maintenance of regular follow-up in adulthood are unknown. The aim of this thesis was to evaluate whether timing of transition affects long-term quality of life and continuity of follow-up of celiac patients diagnosed during childhood/adolescence and then referred to an adult tertiary center. Demographic and clinical data, number of medical consultations during follow-up, and date of last follow-up in clinic were retrospectively collected from patients’ clinical notes. Quality of life was prospectively evaluated by means of a validated questionnaire. These parameters were then studied by means of univariate and multivariate statistical analysis. The results of our study showed that less than one third of patients diagnosed during childhood/adolescence maintained regular follow-up at our adult referral center and that the timing of transition (both age at referral to adult care and duration of disease) is not related to quality of life, and maintenance of regular follow-up during adulthood.
FOLLOW-UP A LUNGO TERMINE E QUALITÀ DELLA VITA DEI PAZIENTI CELIACI DOPO IL PASSAGGIO DALLE CURE PEDIATRICHE A QUELLE ADULTE: ESPERIENZA VENTENNALE DI UN CENTRO DI RIFERIMENTO. Le modalità per un passaggio (‘transition’) ottimale dei pazienti celiaci diagnosticati durante l'infanzia/adolescenza all'assistenza medica dell’adulto, e come queste possano influenzare la qualità della vita a lungo termine e il mantenimento di un regolare follow-up in età adulta sono sconosciute. Lo scopo di questa tesi è valutare se i tempi del cosiddetto ‘transition’ influiscano sulla qualità di vita a lungo termine e sulla continuità del follow-up in età adulta di pazienti celiaci diagnosticati in ambito pediatrico e poi inviati presso un centro di riferimento dell’adulto. I dati clinici e demografici, il numero di visite effettuate e la data dell’ultimo follow-up in clinica sono stati ricavati in maniera retrospettiva dalle cartelle cliniche dei pazienti. I dati sulla qualità di vita sono stati raccolti in maniera prospettica mediante un questionario specifico. Questi parametri sono stati studiati mediante analisi statistica univariata e multivariata. I risultati del nostro studio dimostrano che meno di un terzo dei pazienti diagnosticati durante l’età pediatrica o l’adolescenza mantengono un follow-up regolare presso un centro di riferimento dell’adulto e che il timing del ‘transition’ (età alla prima visita presso il centro dell’adulto e la durata della malattia) non è correlato alla qualità di vita e al mantenimento di un regolare follow-up durante l’età adulta.
