Leriche syndrome, a rare vascular disease, presents a treatment dilemma due to occluded aorta and iliac arteries.This study explores how patient characteristics and anatomy influence the choice between minimally invasive endovascular techniques and traditional open surgery. A cohort of Leriche syndrome patients from a single center underwent either endovascular or open surgical procedures. Outcomes were analyzed in relation to anatomical factors (e.g., aortic calcification) and patient health (comorbidities). Endovascular procedures generally yielded positive outcomes, with faster recovery and shorter hospital stays.However, anatomical limitations in some patients necessitated additional interventions. The study highlights the importance of tailoring surgical approaches based on individual factors. Open surgery might be preferable for patients with severe anatomical involvement despite higher risks,while endovascular techniques could be comparable for less severe cases. The rarity of the disease necessitates further research through multicenter studies and collaborative efforts to refine treatment strategies and potentially develop hybrid procedures combining both approaches. This study emphasizes the need for individualized patient assessment and a multidisciplinary team approach for optimal management of Leriche syndrome.
