Background. Little is known about long-term morbidity and mortality in Whipple’s disease (WD). Aims. To describe morbidity and mortality in patients with WD on a long-term follow-up. Methods. Comorbidities, mortality and causes of death were retrospectively registered. Results. 35 patients with WD (9F, 54±11 years) were followed up for a median of 104 months. Ten patients developed complications and three patients died. 31 severe comorbidities apparently unrelated to WD were found in 20 patients: candidiasis in 10 patients, pre-neoplastic/neoplastic disorders in 7, pneumonia in 4, thromboembolic and cardiovascular events in 8. Conclusion. WD can be complicated by life threatening infectious, neoplastic and thromboembolic disorders. A strict life-long follow-up is required in these patients.
