Equitable access to healthcare, understood as access determined by health need rather than socioeconomic position, is a core objective of the Italian National Health Service. Existing evidence on income-related horizontal inequity in Italy has largely relied on single cross-sections or earlier periods, while recent work documents pro- rich inequity in specialist and diagnostic care in 2023. This thesis extends that evidence over time by estimating a need-adjusted Erreygers concentration index for four healthcare services — blood tests, emergency department visits, specialist visits and specialist diagnostic examinations — using six consecutive annual waves (2018–2023) of the Italian National Institute of Statistics’ Aspetti della Vita Quotidiana survey. Estimates are produced at both the national and regional level, with sampling weights applied consistently throughout the analysis. The results show that need standardisation materially changes the assessment of equity. Blood-test utilisation displays only a weak socioeconomic gradient in the raw data, yet the need-adjusted index is consistently positive, revealing pro-rich inequity that would be understated by the unadjusted distribution alone. Specialist visits and diagnostic examinations display the largest positive need-adjusted indices across the period, while blood tests show smaller but persistent pro-rich inequity; emergency department estimates remain small and negative, with statistical significance vary- ing across waves. For the three programmable services, the 2020–2021 need-adjusted point estimates are generally higher than the corresponding 2019 estimates, although no formal tests of cross-wave differences are conducted. For specialist visits, decomposition of the 2019–2020 point-estimate difference shows that the age contribution becomes less negative, mainly in association with a lower estimated marginal effect of being aged 65 or over, while the prevalence of older respondents changes little. At the regional level, the individual point estimates do not display a clear North–South ordering in the waves examined; however, the analysis does not formally test average differences between macro-areas and therefore does not establish the absence of an underlying territorial gradient. Finally, the weighted descriptive evidence on forgone care shows that cost is more frequently reported than waiting time in 2018– 2019, whereas waiting time is more frequently reported in 2022–2023. Cost also shows a clearer socioeconomic gradient than waiting time. Overall, the findings indicate that pro-rich horizontal inequity in programmable healthcare utilisation is a persistent feature of the Italian SSN context over 2018–2023, while the magnitude and composition of the estimated inequity vary across waves.
L'equità nell'accesso all’assistenza sanitaria, inteso come accesso determinato dal bisogno di salute piuttosto che dalla posizione socioeconomica, è un obiettivo fondamentale del Servizio Sanitario Nazionale italiano. Le evidenze esistenti sulle disuguaglianze orizzontali legate al reddito in Italia si sono basate in gran parte su singole sezioni trasversali o su periodi precedenti, mentre studi recenti documentano una disuguaglianza a favore dei più ricchi nell’assistenza specialistica e diagnostica nel 2023. La presente tesi estende tali evidenze nel tempo stimando un indice di concentrazione di Erreygers corretto in base al bisogno per quattro servizi sanitari — analisi del sangue, visite al pronto soccorso, visite specialistiche ed esami diagnostici specialistici — utilizzando sei ondate annuali consecutive (2018–2023) dell’indagine «Aspetti della Vita Quotidiana» dell’Istituto Nazionale di Statistica italiano. Le stime sono elaborate sia a livello nazionale che regionale, con pesi di campionamento applicati in modo coerente durante tutta l’analisi. I risultati mostrano che la standardizzazione in base al bisogno modifica in modo sostanziale la valutazione dell’equità. L’utilizzo degli esami del sangue mostra solo un debole gradiente socioeconomico nei dati grezzi, tuttavia l’indice corretto in base al bisogno è costantemente positivo, rivelando una disuguaglianza a favore dei più ricchi che risulterebbe sottostimata se si considerasse solo la distribuzione non corretta. Le visite specialistiche e gli esami diagnostici mostrano gli indici corretti in base al bisogno più elevati nel corso del periodo, mentre le analisi del sangue evidenziano una disuguaglianza a favore dei più ricchi più modesta ma persistente; le stime relative al pronto soccorso rimangono modeste e negative, con una significatività statistica che varia da un’ondata all’altra. Per i tre servizi programmabili, le stime puntuali corrette in base al bisogno relative al periodo 2020–2021 sono generalmente superiori alle corrispondenti stime del 2019, sebbene non siano stati condotti test formali sulle differenze tra le diverse ondate. Per le visite specialistiche, la scomposizione della differenza tra le stime puntuali del 2019–2020 mostra che il contributo dell’età diventa meno negativo, principalmente in associazione a un effetto marginale stimato inferiore per chi ha 65 anni o più, mentre la prevalenza degli intervistati più anziani cambia di poco. A livello regionale, le singole stime puntuali non mostrano una chiara gerarchia Nord-Sud nelle ondate esaminate; tuttavia, l’analisi non verifica formalmente le differenze medie tra le macroaree e pertanto non stabilisce l’assenza di un gradiente territoriale sottostante. Infine, i dati descrittivi ponderati relativi alle cure non fruite mostrano che nel periodo 2018–2019 il costo viene segnalato più frequentemente rispetto ai tempi di attesa, mentre nel periodo 2022–2023 sono i tempi di attesa a essere segnalati con maggiore frequenza. Il costo mostra inoltre un gradiente socioeconomico più evidente rispetto al tempo di attesa. Nel complesso, i risultati indicano che la disuguaglianza orizzontale a favore dei più ricchi nell’utilizzo dell’assistenza sanitaria programmabile è una caratteristica persistente del contesto del SSN italiano nel periodo 2018–2023, mentre l’entità e la composizione della disuguaglianza stimata variano da un’ondata all’altra.
