The glomerular filtration barrier (GFB) is a fundamental structure in the renal system responsible for blood filtration. It is composed of podocytes, glomerular endothelial cells (GEnCs), and the glomerular basement membrane (GBM). The majority of current 3D in vitro systems cannot faithfully mimic this structure because of its complex three-dimensional architecture and biomechanical characteristics. This study aimed to develop a three-dimensional millifluidic glomerulus-on-chip (GoC) platform based on human induced pluripotent stem cell (hiPSC)-derived podocytes and endothelial cells cultured on opposite sides of a coated porous membrane under static and dynamic conditions. Computational fluid dynamics (CFD) simulations identified laminar flow throughout the device and wall shear stress (WSS) values on the membrane ranging from 10^-4 to 10^-3 dyne/cm^2. Co-culture was associated with increased NPHS1 expression and translocation of nephrin from the perinuclear region toward the cytoplasm and cell membrane. Under flow conditions, ZO-1 and nephrin showed a continuous junctional localization and a proper cell border localization, respectively. GBM-associated proteins COL4A5 and LAMA5 remained localized within the extracellular matrix in both podocyte and endothelial compartments, although their transcript levels differed across culture conditions. Endothelial cells showed a well-preserved VE-cadherin localization and reduced ICAM-1 expression under dynamic culture conditions. These findings indicate that the combined effects of endothelial-podocyte crosstalk and controlled fluid flow were associated with changes in slit diaphragm and junctional organization and maintenance of endothelial structural integrity. The platform reproduces several features of the human GFB and represents a useful tool to investigate its physiology and disease mechanisms. The use of hiPSC-derived cells may also enable patient-specific modelling of glomerular diseases.
La barriera di filtrazione glomerulare (GFB) è una struttura fondamentale nel sistema renale, responsabile della filtrazione del sangue. È composta da podociti, cellule endoteliali glomerulari (GEnCs) e dalla membrana basale glomerulare (GBM). La maggior parte degli attuali sistemi 3D in vitro non è in grado di riprodurre fedelmente questa struttura a causa della sua complessa architettura tridimensionale e delle sue caratteristiche biomeccaniche. Lo scopo di questo studio era sviluppare una piattaforma millifluidica tridimensionale glomerulus-on-chip (GoC) basata su podociti e cellule endoteliali derivati da cellule staminali pluripotenti indotte umane (hiPSC), coltivati sui lati opposti di una membrana porosa trattata, in condizioni statiche e dinamiche. Le simulazioni di fluidodinamica computazionale (CFD) hanno identificato un flusso laminare all'interno del chip e valori di stress di taglio sulla parete (WSS) sulla membrana compresi tra 10^-4 e 10^-3 dyne/cm^2. La cocultura è stata associata a un aumento dell’espressione di NPHS1 e alla traslocazione della nefrina dalla zona perinucleare verso il citoplasma e la membrana cellulare. In condizioni di flusso, ZO-1 e nefrina hanno mostrato rispettivamente una localizzazione giunzionale continua e una corretta localizzazione a livello dei bordi cellulari. Le proteine associate alla GBM, COL4A5 e LAMA5, sono rimaste localizzate all’interno della matrice extracellulare sia nel compartimento dei podociti che in quello endoteliale, sebbene i loro livelli di trascrizione differissero tra le diverse condizioni di coltura. Le cellule endoteliali hanno mostrato una localizzazione ben preservata della VE-caderina e una ridotta espressione di ICAM-1 in condizioni di coltura dinamica. Questi risultati indicano che gli effetti combinati dell’interazione tra endotelio e podociti e del flusso controllato sono stati associati a cambiamenti nell’organizzazione dello slit diaphragm e delle giunzioni cellulari, nonché al mantenimento dell’integrità strutturale dell’endotelio. La piattaforma riproduce diverse caratteristiche chiave della GFB umana e rappresenta uno strumento utile per studiarne la fisiologia e i meccanismi patologici. L'utilizzo di cellule derivate da hiPSC potrebbe inoltre consentire lo sviluppo di modelli paziente-specifici di glomerulopatie.
